Not recieved my free 6months DTH connection

I have purchased LCD TV Monitor Model No- M227WA, Serial No.- 908NLBU005357 from one of LG SHOPPE Electronics. on 2 Sep 2009

With this product i have also got the promotional offer of DTH SET TOP

BOX with six month subscription for free, and for that i have filled

the application form of spectramind marketing ltd having S No.320445

and send the same with all necessary required documents and a demand

draft of Rs 99 to them on 15/09 via FLYKING COURIER service.

Unfortunately, i have not recieved the set top box and subscription

till date. Please look into the matter and send the same as early as

possible.
