Non-receipt of TATA SKY set-top box

I have purchased a 22" LG Monitor TV from M/s. Harivallabh Infocom, Ahmedabad on 29-6-09. I have already couriered the necessary documents asked for the free Tata Sky set-top box along with 6 months free subscription. I have couriered the documents on 3-7-09 and have received the delivered message from the courier company also. The deatils of my purchase are as follows: invoice no, T/09-10/891 dated 29-06-09 Model no. M227WA-PT.ATRZ Serial no. 906nlnc000703 DD of Rs. 99/- DD No. 897420 claim voucher no.203387 kindly arrange for the delivery.
