Sir,

I am the holder of above mentioned policy no who’s Table no is 122-09, The pension for this policy was schedule to start in March. I have received the option form in Feb which I have deposited in your branch by hand on 2/3.

I have given “F” option on ECS credit mode, but on checking my Bank a/c I am surprised not to see any entry from LIC of Pension opted.

Kindly do the needful to credit the amount I owe, as this has taken very long to credit the promise made by LIC to me.
