Dear Sir,

while paying premium on 5/03/09 of policy no. 861841474, 967511080, 967515640, 967515782

for the month of March09 first transition failed & then tried to pay again both (two time) time amount Rs. 2622/- deducted. Not refunded till now & not getting receipt of premium paid

please take corrective action
