Non-receipt of Maturity Payment

On 14.1.04 I have started the Jeewan Suraksha Policy (Option one time maturity payment) at LIC Branch Dalli-Rajhara (Dt-Durg, Chhattisgarh). at the age of 45 yrs. As per the terms at the age of 50 yrs the policy will be matured and one time maturity will be paid to me. The policy have been matured on 13.1.09. The documents has been submitted at the above said LIC branch. The one time option has already been mentioned in the policy; whereas after submitting documents the branch is expressing that the option form is to be submitted again and the form is not available at the branch, which is supposed to reach from Bhopal Regional Office.
I am lossing the interest on Rs. 50000/- interest / Bonus since 13.1.09.
Kindly look into the matter immediately so that my mental agony be reduced.
