My Pension Policy No. is 250193492.The annuity payment is by LIC of India, Jeevan Akshay cell, North central Zonal Office 16/98 MG Road, Kanpur208001.NO annuity payment has been credited to my bankers PNB, sector 18 Noida sb a/c since o1 Sep 2009.The required life certificate duly signed was depositedwith LIC Branch12g Nehru place, new delhi110019 on 15 Sep 09.Could you kindly commence the annuity payment?

if the requirement of life certificate is going to be regular annual feature the form should be sent to me by Aug rather than annuity being withheld
