Sub Non Deduction of Premium from salary

Ref Policy No 115762082 in the name of Bushi Sathyan for Rs.300000/- dated 17/03/09

With reference to above I like to inform you that I have take above mentioned policy from your Delhi Branch Under Salary Saving Scheme, through agent Mr Rajinder. I have filled all the application for Salary deduction. But iam really sorry to inform you that so far the Installments are not deducted from my salary. Presently iam transferred to Federal Bank, Br Kollam, Kerala State.

So kindly find out the reason behind non deduction of premium from my salary, and please be informed the porceduture for transferring the mentioned policy to LIC of India Kollam branch for future service, and the arrears in the mentioned policy as on date.

Please treat the matter most urgent and do the needful to avoid the policy getting lapsed.
