I have been working with Ace glass containers between 1994 and had been in superannuation scheme offered by the company with the master policy no. GSCA -303775. Recently, I have received the annuity payment for Rs.6426/- from your dept to my home address for the annuity no: 011Y 0219078 and I have lost the same without being deposited in my bank account.

I request you to kindly order stop payment to the cheque issued and issue a fresh cheque in my favour.

I also request you to send the relevant documents to sign and send it to you for electronic bank clearance in future.
