PF withdrawal status is not given, chaque not recieved yet

Dear sir,

I release Ma foi Management Consultants ltd. pay roll on 31-jan-05 and submitt form for EPF withdrawal. I recieved chaque return mamo for the icici bank stating mismatch in my name, but didn't recieved chaque along with. then I recieved letter from EPF organisation on 19-july-05 requesting to provide bank pass book copy to ex-employer for name verification which I also submitted on time.

But till date I didn't recieved any response from that organisation after doing numbers of follow up.

My EPF No:1222662. Kindly let me know the status of the same.
