Withdraw of P.F. Amount

Sir/Madam,

I am working in Govt. Dept. right now but was working with D.L.F. Ltd for 5 months. I left co. without giving any prior notice. How can i get my P.F. amount from that co.? I am having Joining letter, all Salary slips with P.F.A/c no.
