Dear sir,

i am paid rs.2500/-to max new york for taking a policy by cq.no.057126 dated 26/10 of icici bank ltd goregaon br, mumbai and the same was cleared on 26/10.

as after paying above amount no comunication from company for issuing policy or policy premium remander till dated 09/05.then i am applied for refund of payment paid by me. on15/05 i am socked the company is deliver the laps policy and refuse for refund.
