Non-delivery of medical documents

This letter is to bring your kind attention to following important

fact. I had given 2 important medical documents to one of your offices at Malaparamba, Calicut (Three Star Agencies, 11/63/C-7, Roopa Plaza, Calicut-9, phone No. 0495-3247774) on 28/08/08 (receipt No 9099387 & 9099388). These were 2 important medical documents to Dr. Ajith Awasthi, Dept. of Psychiatry, PGIMER, Chandigarh.

Till date these 2 documents have not reached the destination and is missing. Your local office is not able to give any clear explanation for the same. These are very important documents and I have to get very clear answer from your office as what happened to these documents. If it is delivered I would request you to immediately fax or e-mail me the scanned copy of the signed and sealed receipt of the from receivers side as the proof of evidence.

If not you have to give me adequate explanation for the irresponsible behaviour and the resultant losses which has happened to me. I need immediate reply for this message because my future depends on these 2 letters. I am also attaching the copies of the receipts which I received from your local office.
