Cheating, Copying my signature on separate form

I Sachin Taneja holder of Indian Passport number G9277693 opened a metlife policy by Metlife Agent Gaurav sehgal on 2nd dec. I filled the form myself and i ticked single time premium option. But now i came to know that my policy has been started for annual premium of 50, 000.The policy i rcvd has xerox copy of form which is not filled by me even signature on that form are not mine they have been copied by some metlife agent which don't match my signature. Lot of other mistakes have been made in new form. I am Non resident indian i was away from my indian address when the policy was delivered at my home and now when i saw all this i complaint in Metlife insurance but no response from their side. I want i have to be given the same one time premium policy no IRDA rules will be destroyed.

There is a witness signature of Axisbank emloyee near my signature with the stamp but Axisbank Ambedkar road Ghaziabad is unable to provide me details of that employee who has witnessed.
