My connection have disconnected

Hi Sir,

I have purchage a new phone plus connection to relience web word to Southe Ex Baranch New Delhi. Problem Is that in the Purchaging time I have fill the all form and also Attached the photo copy of Drivering Lisence and submit Relience Executive after 10 days my connection have disconected due to reason was application form not Submit. I have allready sumbit all kinds of document then they had activate the connection why i will submit my document again..I am very Upset. So Please Resolve the Issue ASAP.I have Purchage mobile on 29 june and My phone have Disconnected 13/july. I am official gues i have not time to visit Relience web word.
