Billing of crdit shield premium without authorisation

The Bank has applied credit sheld facility with out my approval. On cmaplaint the policy has been withdrawn, but the premium for the period for which the policy was applied still levied on to my account. It is unfair practice to apply such policy without written concent of the card holder.
