Sub: PF Account No. WB/33162/Z20 standing in the name of my brother Lourdu Royan who expired on 29-5.

My brother Lourdu Royan alias Singa Rayon worked in M/s. Rupa & Company as a Sales Executive. During this time he became a member of Employees' Provident Fund vide the account No. mentioned supra. Subsequently, he left this company during the year 2003. But so far he has not withdrawn the amount. Unfortunately, my brother expired on 29-5. I request that the amount of Provident Fund standing to the credit of my late brother may be settled in favour of his widow Smt. A.Harriet alias Sarika, Aged about 32 years and minor son Master Martin Joseph aged about 5 years, R/o.1-7-510/E/2, Harinagar, Zamisthanpur, Hyderabad - 500 020, Phone No.040-27664506.I also request that monthly pension as deemed fit and proper may be sanctioned.

The widow of Late Lourdu Royan has no source of income whatsoever. She has to look after her minor son Master Martin Joseph and also educate him. Your help in this regard would greatly help her, for which kind of kindness she would ever be grateful to you, Sir.
