Transfer of Account

I am having an account in ICICI, Bhilai branch No. 018601 506617 and wanted to transfer in Secunderabad branch as I have left Bhilai and stationed in secundrabad, I have submitted an application along with my cheque book etc to ICICI bank, Secunderabad branch on 11.08.08. It has almost 2 weeks over neither I have got any information nor cheque book so far. Pl get examined and expedite an immediate action for transfer and activation so that I would be able to operate my account.
