Non receipt of claim against Form C

Dear Sir/Madam,

My last employer, Terex Vectra Equipment Pvt Ltd. had duly submitted my PF withdrawal forms (Forms No.19 & 10 C) to the Regional Provident Fund Commissioner in December. The PF Account No. is "DL/28179/034". By September though I recieved claim for Form No. 19 but No claim against Form 10C has been recieved till date (July). For your information ref. 28717535 dt. 24th Sep 08

I recieved a letter asking me to submit proof of my birth certificate which was submitted in October again it was informed to bring original Passport & Class X th certificate only then my application would be recieved finally in November 08 as per satisfaction of PF office Form 10c was again submitted at Nehru Place PF office. Now I am being told to submit proof of my death certificate when I confirmed I am alive it was told I have to resubmit form as it has been sent back to my previous employer and it has been lost.

Can I know how do I get this claim in my account now?
