Sir,

On 15.09.09 I filled NPS registration form at ICICI bank, WaterField Rd, Bandra-W branch. The dealing officer Mr. Ujjwal (ph-9920766326).

But, I ahve not yet alloted registration identity till date.

My details are as follows:

PRAN: 11600195100000017 / 18

POP-Reg no : 6001951

All my attempts to follow up my subsriptions are futile.
