NHPC IPO
NON RECEIPT OF REFUND ORDER

I beg to state that I have applied for 700 numbers of equity share of M/s. NHPC - IPO. My application no. was 8-6375903. I have been allotted 190 numbers of equity share of the above IPO. My demat account with PNB: New Market, Kolkata (0093) and my client ID No. is 10200572 and DPID-IN 300708.

My earlier bank a/c no. was 50050 with PNB, Clive Row branch, Kolkata and my mailing address was Flat no.2/c, 420 Dum Dum Park, Kolkata – 55. On my transfer from Kolkata to PNB, Circle Office, Midnapur, I have closed the above said OD a/c and my new CBS a/c no. with PNB: Midnapur is 0350009400020449.

My new mailing address is C/o. Sri Subrata Kumar Das, Village – Nuapur, PO-Kalyani via Gopalpur, Dist.-Balasore, Orissa, Pin no.-756044.

On my request, recently my depositor has already changed my mailing address and the bank a/c number. The above address and bank account no. has been incorporated in my above Demand Account.

My refund order for NHPC IPO has been sent to my old address at Kolkata. The above warrant may be returned due to non delivery. So I request you kindly send my refund order of the above IPO in my following new address:
