Monthly statement not recieved

Hi

past from 6 month i haven't recieved my statement from axis bank.

This bank service is very bad and careless. I given my permanent adress, office adress and e-mail id also. But still i wouldn't get my statement. Very shameless people working in this organaisation. If u have some responsibility please send my statement.
