Claim of CPF amount

Sir,

I would like to take a suggestion from you regarding claim of my PF. I had worked in NAL, an autonomous govt. organization from june till september. In NAL my PF subscription was GFP. On october I had joined NIMHANS, an autonomous organization and I worked there for one year till October. In NIMHANS, my PF subscription was CPF. Due to personal reasons, I have to join back to my original institution, and joined NAL for duty on November. Now, I wanted to Transfer my PF amount from NIMHANS to NAL PF account. But, the authority of NIMHANS said it cannot be transferred. Then I thought that I can claim my CPF amount from NIMHANS, but they are telling that I cannot claim the CPF amount, since I am working in another govt. institution. I do not know how to claim my CPF amount from NIMHANS. Kindly give me a suggestion and form numbers which I have to apply to claim the amount.
