PF NOT CREDIT IN MY A/C

Dear Sir/Madam,

I Arjun kumar madheshiya my pf no.is KN/BN/34272/40128 i have already submit the all document for withdrawl my pf as on date 04/04 and adecco flexione submit my pf form to pf office as on date 19/11 but still my pf amt.not credit in my bank a/c and i am trying to contact in pf office many time yet not given any possitive response.

So kindly request to you please help me and take the action about this matter and reply my status asap,
