Transfer not yet done. missing annexure

Hello,

Last year the PF cheque for account AP/HY/35709/1037 was sent to Mumbai with the amount Rs. 113256, but the Mumbai office did not receive the annexure K witir, h the cheque.

So the Mumbai PF office sent an application to you for duplicate annexure on in April.

Please let me know if you have received the application as I made several contacts to Hyderabad PF office but could not get proper details.

If you have received the application, please send the duplicate annexure ASAP. I request your help as its almost 2 years and I am in need of money as I am jobless.
