Unable to print PDF form

Dear Sir/Madam,

I applied for fresh passport online. And i got appointment date on 14-01. But i am unable to confirm the given date. Because in confirming page dates are not displayed in date field for month january. but for month february it is displaying. i tried for so many times in different browsers. But the situation is same. Could you help out me regarding in this. I have appointment date on 14-01.
