No receipt of pension last eigth years

Respected sir,

my cousin brother late shri Ghanshyam Singh was employee of oceanglobe container ltd till march when he died in accident. He worked as storekeeper in the company. For the pension, pension form duly filled up along with all required documents were submitted through the company to the pension office in mumbai. The pension office made us submit pension form thrice on the pretext that the pension form was mutilated or lost. The lat time it was submitted on 02/11. Following are the receipt details:

Sir,

The widow of Shri Ghanshyam Singh is very poor and to look after two wards and take care of their education. Without any source of income it is now difficult even to survive. Inspite of submitting all documents thrice authorities at pension office has not bother at all to commence the pension. On behalf of the widow Smt Sushila Singh, I request you to kindly look in the matter and do justice to the poor lady.
