Closed my account and widrawal amount

It is requested that please colsed my account and paid my amount to send in my account of SBI .I have open my account in Gandhidham (Gujarat) your concern in april and deposit annual amount 9100/- per year, policy no. u143475398 . I want to closed my account for some domestic problem. I have already deposited all document with 20/- stamp paper. pse closed my account and paid my amount in my SBI account No. 30035076272 at Bank Rewari (Haryana).
