PF and Gratuity

I had resigned from PAM Consultants, Ahmedabad in July. In this company i was entitled for PF and Gratuity as i had completed 5 years employment in the company. My employer was forcing me to draft a letter according to his suggested format. I refused to do so because it was contained some unreal information about my employment period. So, he had put my Providend Fund and Gratuity both on hold whereas he has already got my gratuity cheque from the respective government department. Now it is almost a year but still he didnt processed my PF form and not also delivered my Gratuity cheque. Please someone suggest me a proper way to get my PF and Gratuity back.
