NAME IS INCORRECT

I have recieve my pan number BEJPK4639F in the month of February but in my card my name is written incorrectly. It is written CHANGAPPA P C CHENGAPPA but my correct name is KRISHNAPRASAD P C.

I will be hieghly greatfull for making neccessary changes and correct card send it immediately.

I am requesting to send my PAN card in the address given below
