Wrongly printed surname

Dear Sir,

i appllied for a new pan card on 19th February vide application No: 045161701 and Coupon No:031378191 and when i checked the status of pancard on computer after 15 days my name on computer displayed as JOHNDHARMENDRA ALLADA but on my pancard the surname is wrongly printed it is printed as ALLA JOHNDHARMENDRA instead of ALLADA JOHN DHARMENDRA.

i want to know the procedure to rectify my correct surname on the pancard kindly tell what to do?
