I HAVE APPLIED FOR NEW PAN CARD. I GOT AKNOWELDGEMENT NO: 021140100038831. Once I got information that proof of identy not provided/not valid., even though I have submitted along with application. I once again sent the proof of identy (copy of driving licence) between 29.04.09 and 5.05.09 (exact date not remembered).

Request you to arrange to send the pan card at the earliest.
