Make pancard twice

I was apply pan card in Dec i got pan no. through internet AKLPD5751E but i was not received pan card, after inquiry they said"this card dispatch from here all mistake by delivery company" then i asked them what i do? they said me apply for recalling.therefore i recalling this card i fill the form in June they dispatched it with name mistake "Deore Balasaheb Deore", i send it back for correction "Kapil Balasaheb Deore" but from your staff dispatched new one card BBJPK3263N, and it was also not send to me. finally i have not got any card i complaint to income tax office they said me give your any pan card, but i have not any pan card, then what i give them. please help me. which document you want i give but give me one car and cancel other one.
