Sir,

I have not received my PAN Card No. ANXPA8448A allotted by Income Tax Deptt. when I checked the status thru NSDL website there shown that my PAN Card has been delivered to any Mr. Madan (Secretary) on dated. 22nd Dec.2008 by Expressit Logistics but I don't know any Mr. Madan in my Shop. It seems that my PAN Card has been delivered to wrong person who may by misuse it. If this will happened then courier company will be responsible for any mishappening.

I request you please arrange to deliver my PAN Card to me at my right address as below:
