I had worked at Konark Paper & Industries Ltd. situated at DIST - MAYURBHANJ, JHARIA - 757016, ORISSA. My Account No. was "OR/3070/698". Now the the company is closed and I want to withdraw my Provident Fund. In Form No. 10-C of Employees' Pension Scheme, 1995 they want the following details for which I have no idea.

3. Estt. Code No. -

4. A/c No. - OR/3070/698 (IS IT CORRECT ANSWER?)
