I had already forwarded my attested Ration Card photocopy and bank passbook photocopy to your office once on 28 Aug and again on 10 Nov.

But the status is still showing as stated above.

(a) Furnish attested copy of proof of your current address i.e. Election Comm. Identity Card/Ration Card/Elect. or Telephone Bill/Bank Passbook (all pages)/service certificate from employer etc.which should be in your name/spouse's name or in the name of parents

I am requesting you to please look into this matter and kindly issue my passport.

A quick action will be highly appreciated.
