Hi,

While leaving my previous organisation - Patni Computer Systems Ltd, Mumbai, I applied for closure of my P.F A/C and refund the amount to A/C which I mentioned in the form.

P.F A/C No - MH1873426577.

Date of leaving the organisation - 16th June
