Name alloted erraoneously

I have applied for Pan Card in the year 2005 through my C.A. I received my pan card with Pan No. ADMPM6574A But my name was written "V.VASUDEVA RAO" INSTED OF "M.VASUDEVA RAO'. After that I have returned to my C.A. for rectification. Then the Card was misplaced and now traced out. Kindly guide me How to rectify my name.
