Personal loan in my name which i never take

Recently I had applied for cash credit limit which was not approved by PNB on a/c of some Personal Loan default showing in my name but I never applied for any personal loan from any bank up to till now. Interestingly Bankers name, account detail and other relevant details are not appearing in the CBIL report from which I could understand which bank has put my name in defaulter list.
Can any one guide me how to get the detail from CBIL so that the respective financial institution can contacted for clearance.
