My pf is not settled

Myself Lordumanickaraj wwas a employee of pharmapil, was working from june 17th to 31 march. My pf no was AP/KKP/59218/150. I submit my pf settlement form as per trichy pf branch officer advice to my company several times, i was not received. Kindly take immediate action about this petision.
