Not being provided with ATM

This is to inform you that I had got opened a salary account (account.no- 3914000400190803) at Punjab National Bank (KNIT Branch), Sultanpur (U.P.) on 06/Oct/09. Even after more than 20 days & several requests the bank is not providing me with the ATM & is neither accepting my ATM request form, making silly excuses that your signature has not been scanned & is not loaded on the computer and will be scanned after we will get time. However these excuses are fake because the bank rule says that the customers should be provided with the ATM facility immediately after getting an account opened.

Even after I had received my passbook I am facing problem in withdrawing my salary.

Kindly help me out and provide me with the ATM.
