Sim is not working

My mobile phone which is a lifetime prepaid connection. had not given problems in the past two years. Last month my phone no.9952042751 was disconnected given a message ask puk number, wherein when i had taken the sim i went that shope they told u r sim is damage so u change new u will get same number then ok i pay RS250 i bought same number sim ofter two days again sim blacked. again msg limited service i dont know why they do ing like this till my sim not working.
