Forged Signatures

I applied for an Airtel connection in April 09.

In June they returned my form stating that no connectivity is possible in our area, where as just above my flat there is airtel connection.

They misplaced my cheque. They returned my signed form. I am shocked to see that there are total 2 more pages added to the papers i signed. Both have my forged signatures. I signed the whole form is black, these signatures are with blue ink. There is a proper stamp and signatures of Airtel reference numbers on the papers.

I wrote an email to nodal and appalette, no revert from them. I tried calling them, no response.

This is a sheer case of forgery. I need to file a legal case now. Please advice.
