Wrong Name in the mobile no

Dear Consumer Forum,

I have taken a airtel no 9958808625 in the month on June 09 When i received the bill, it was billed on the name of my company SBI life insurance co ltd. I requested the airtel company many times, by visiting their distributor as well as sending them emails, but still they have not corrected the name. I am afraid that they have changed the application form and my signatures too. Also today i again received bill for the current month also in the name of my company.

I have been requesting them to provide me the copy of application form, which they denied.

Kindly support in the matter.
