Non reciept of Passport

I Er. Dharam Parkash Kaushal had applied the passport on 17-02 vide File # CHDA00998209 but after more than three and a half month passed my passport has not been sent. Yesterday only I got a letter from Regional Passport office, Chandigarh through ordinary post (written to me on 8th May) that they require death certificate of my father who was passed away more than 40 years back. I have dropped mail to RPO but nthing has happened. After long trial I could be able to connect my phone line to no 01722606889 and the person on line said the letter was posted mistakenly, actually recent photographs of mine are required though I have submitted my recent photographs. It is really a heights of negligence and a common man suffers.
