Trnasuction declined but amount debited from my acount

Sir, I have an savings account in Andhra Bank, Buxi bazar, cuttack, orissa, I regularly draw money from different Bank ATM Counters, on 28/07 I visited the AXIS Bank ATM Centre at Mahanadi vihar, Cuttack, Orissa, when I inserted the card and after typed my Pin number the report came out that your has declined but it surprised I saw that the amount of Rs 4000/- has been deducted from my account.

I request the concerned authorities to look into the matter and see that my amount of Rs 4000/- will be credited again to my account.
