Non-receipt of provisional pass certificate and mark sheet

I have completed my B.com, i got my result on 16-010 but till now i have not received my mark sheet and provisional certificate. Since because of this i am loosing so many good job opportunity. I get not response through phone when i am trying to contact then. I have mentioned my details below please help me to fix this issue.
