Sub: PF Settlement not received

Reference: PF NO: AP/42802/587. Organization -FAMILY HEALTH PLAN LTD

Bank A/c No : 04220040003331 – Kotak Bank- Lavelle Road Branch-Bangalore

I wish to bring to your notice that I had applied for PF settlement in the month of April and the settlement has not been done so far.

On Enquiring about the status of my claim, I am informed that settlement has been done but A/C No. mentioned in the cheque is incomplete, last 2 digits are missing. Where as I have mentioned my complete A/C No in the PF application. I have been trying to coordinate with PF Office but I am not getting a proper reply.

Two weeks ago, I received confirmation from my HR Dept. Stating Cheque was sent from Hyderabad Provident fund office on 16-6-08.

Cheque details are as follows

Cheq. No: 802250 for Rs.10797

Cheq. No: 840247 for Rs.23735 of SBI Bank.

On discussing with the bank, they have confirmed that they have not received any cheques.

I therefore request your good selves to please help me in getting my PF amount at the earliest.
