Non-receipt of Pension Scheme Certificate

Sub: Non-receipt of Pension Scheme Certificate

I had applied for Pension Scheme Certificate in February through my old employer to the Regional P.F. Commissioner, Indore and I received a letter No. Sec. III-M.P. 10623/1390/1036 dtd 6/3 from the PF Deptt. for providing certain details, which I had provided along with my previous Scheme Certificate in original on 20.03.2009 through my ex-employer with a request to issue me a combined Scheme Certificate. After producing required details, I have not heard anything from PF Deptt. evenafter sending reminders through fax as well as by hand nor received the Scheme Certificate.

As already almost a year has passed after submitting my application, I am knocking your door for justice and requesting you to please take up my case at appropriate authority and arrange to issue me a Combined Scheme Certificate, at the earliest.
