Non receipt of provident fund amouy on its withdrwal

Respected Sir,

Sir,

I had submitted the PF withdrawal form through my former employers M/s. Ashapura Mine chemicals Ltd during the year 2009 and till date I have not received any amount or communication from the Bhandra OF Office.

On enquiry by my employer it was told that my settlement amount has been wrngly credited to some other bank account number.

My PF Account No. is -MH/ 41243/525

Bank account no is -0804101055635

Now as of today, almost two tears passed by and residing at Kerala so that frequent follow up is not practical due to my age and health problems.

Therefore I have no other alternative left than to request yoyr goodself to pass approiate order or steps to save me from this situation.

I once again humbly request your goodselves to kindly expedite the same, without any further delay.
