PF withdrawal amount not received yet - even after 4 months of submission

I, Jayalakshmi Kumaresan, was employed with Indegene Lifesystems Pvt. Ltd. for the period 1st October to 4th May.

This is regarding my EPF & EPS withdrawal post my leaving the services from Indegene Lifesystems Pvt. Ltd. The withdrawal forms were submitted on 16.09.2009 at the below address:

2nd Floor, CMC Building, Begur Road, Bommanahally,

Bangalore – 560 068

When I'd enquired about a month back, I was told that the withdrawal amount will get credited into my savings bank account (mentioned in the form) by the 1st or 2nd week of January. I've checked with my bank last week & again today. The amount is not credited yet. My PF account no. is given below:

KN/BN/24781/69

Kindly help me in getting my PF amount at the earliest.
